Impact Soccer Academy 

 

RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT 2025

 

I understand Impact Soccer Academy and their staffs are not responsible for any injuries incurred, or any loss or theft of articles resulting as a part of participation in or travel to and from the Impact Soccer Academy Program. In consideration of Impact Soccer Academy allowing my participation in its Skills Program, I agree to the following: 

 

1. To waive all claims that I have or may have in the future against Impact Soccer Academy, its members, employees, volunteers, and independent contractors (all of whom are hereinafter collectively referred to as “Impact Soccer Academy”). 

 

2. To release Impact Soccer Academy from any and all liability for any loss, damage, injury, or expense that I may suffer, or that my next of kin may suffer as a result of my participation in the activity of soccer skills training, due to any cause whatsoever including negligence, breach of contract, or breach of a statutory or other duty of care. 

 

3. To hold harmless and indemnify Impact Soccer Academy from any and all liability for any damage to the property of, or personal injury to, any third party resulting from my participation in a skills program. 

 

4. This agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns, and representatives in the event of my death or incapacity. 

 

5. I, the undersigned, hereby authorize Impact Soccer Academy to photograph my son/daughter, take motion pictures of, take video footage, and/or make electronic sound recordings. I understand that these pictures and/or videos may be posted on Impact Soccer Academy social media accounts including Facebook, Instagram, Twitter and Snapchat. 

 

In entering into this Agreement, I am not relying upon any oral or written representations or statements made by Impact Soccer Academy other than what is set forth in this Agreement. 

 

I have read and understood this agreement and I am aware that by signing this agreement I am waiving certain legal rights which I, or my heirs, next of kin, executors, administrators, and assigns may have against Impact Soccer Academy.
